Inclusive and Affirming Ministries (IAM) Reporting Form

At IAM, we are committed to fostering a safe, ethical, and transparent environment. We take all reports of abuse, fraud, bribery, or other wrongdoing seriously. This confidential reporting mechanism is available to both internal and external stakeholders who wish to report concerns safely and anonymously (if preferred).

1. REPORTING OPTIONS

You can report your concern through any of the following methods:

• In-Person: By request, we can arrange a confidential meeting.

All reports will be handled with the strictest confidentiality. You may choose to remain anonymous, but providing your contact information allows us to follow up if needed.

• IAM ensures that all reports are treated confidentially and shared only with authorized personnel responsible for investigating the matter.
• We prohibit retaliation against anyone who reports a concern in good faith.
• If you fear immediate harm, please contact local authorities or emergency services.

4. NEXT STEPS & FOLLOW-UP

• Your report will be reviewed within 5 business days by our Safeguarding & Ethics Committee.
• If you have provided contact details, we may follow up for further information.
• You will receive updates on the progress and resolution of the case (where legally possible).
